Location: REMOTE / Toronto, Ontario
This job allows you to work remotely.
The client is a healthcare AI company that turns routine lab results into clinically validated chronic disease risk predictions. Their algorithm is CE marked in Europe through a partnership with a global diagnostics company and is being embedded into specialty care workflows through a second strategic partnership. They closed a funding round in the past year and are building out their engineering team.
THE OPPORTUNITY
This is a hands-on, senior individual contributor role reporting to the Head of Engineering. You own the build of a brand-new population health platform from day one. Nothing exists yet beyond a rough prototype, so you will help choose the stack, design the architecture, and write most of the code.
The core of the platform is getting clinical data in. Patient data arrives from clinics and partner organizations through EMR integrations (HL7, FHIR) and large bulk uploads of hundreds of thousands of patient records at a time. You will build the ingestion, cleaning and de-duplication so that data lands correctly in the platform, then build what sits on top of it: the APIs, the database, and the user-facing app that surfaces the patients most at risk.
You will work closely with the company's new product lead to turn clinical and business needs into user stories, technical specs and an MVP targeted for Q1. A QA/DevOps engineer owns the pipelines. As the team grows you will mentor more junior developers through pairing and code review.
WHAT SUCCESS LOOKS LIKE
• A stack and architecture decision you can defend, made in your first month (front end likely React, backend open, with Java/Spring Boot a leading option for FHIR).
• Reliable ingestion of large clinical datasets with validation, cleaning and de-duplication built in.
• Working EMR integrations over HL7 and FHIR.
• An MVP of the population health platform delivered with the product lead in Q1.
DAY TO DAY
• About 90% technical: architecture-heavy at the start, then mostly hands-on coding.
• Own features end to end: data model, API and UI.
• Break user stories into technical specs with the product lead.
• Handle health data (PHI) in line with privacy and security requirements.
• No direct reports for now. Light, self-managed project management.
LOCATION AND COMPENSATION
Toronto preferred, then Winnipeg, then remote anywhere in Canada. $180,000 to $220,000 CAD base plus ESOP participation and benefits.
Must Have Skills:
• 8+ years of professional software development, building and shipping production web applications end to end (front end, back end, database, APIs).
• Hands-on EMR integration experience with HL7 and/or FHIR.
• Experience ingesting, cleaning and de-duplicating large datasets (hundreds of thousands of records or more).
• Has built a product from scratch at a smaller company or startup and made the core architecture and stack decisions.
• Strong system design and data modeling judgment, and can explain the trade-offs.
• Works directly with product management on requirements and communicates clearly with technical and non-technical people.
Nice to Have Skills:
• Shipped software regulated as a medical device (SaMD) under IEC 62304 / ISO 13485 design controls (strongly preferred).
• Java and Spring Boot, especially with FHIR libraries (HAPI FHIR or similar).
• React.
• Azure and modern CI/CD.
• Putting ML models into production (versioning, monitoring, retraining).
• Multi-tenant or role-based SaaS.
• Health data privacy and security (PHIPA, HIPAA, GDPR, SOC 2).
• Mentoring junior developers through pairing and code review.
• Uses AI-assisted development tools on production code.
